Peace in Cognitive Wellness PLLC is committed to helping you understand your patient rights—including (a) how to request medical records, (b) contact the state’s Executive Council, and (c) file a consumer complaint.

This notice has been written in accordance with the Health Insurance Portability and Accountability Act (HIPAA), House Bill 4224 (89th Regular Session), Section 181.105 of the Texas Health and Safety Code. If you have any questions about this bill, please visit the Council's Contact Us webpage.



SECTION A: REQUESTING MEDICAL RECORDS

You have the right to request a copy of your medical records (also called healthcare, mental health, or clinical records) for your own use or to be released to an outside party.

Current Clients: Please submit your request via email at Chambers@peaceincognitivewellness.com.

Former Clients: Please submit your requests via email and include your full name, date of birth, and details of your records request. Requests may require identity verification.

Q: How long will it take to receive a response? Our office typically responds within 24 business hours. If we are closed for holidays or scheduled time off, we will reply as soon as possible, and no later than 14 days. Requests for records are processed promptly, but please allow up to 15 business days from the date we receive your request.

Q: Can my medical records request be denied?
Yes. In some cases, we may be unable to share records due to legal restrictions or if releasing them could cause harm. If your request is denied, you will receive a written explanation within 15 business days. Whenever possible, we will provide alternative information, such as a summary or letter regarding your care.

Q: What can I do if my request is denied?
You may request a review of the decision by a licensed healthcare professional not involved in your care. To do so, contact our office within 30 days of receiving your denial letter. You may also file a complaint with our licensing board or with the Texas Attorney General’s Consumer Protection Division (see SECTION C for instructions).

Q: Can my records be sent to my doctor, psychiatrist, or another provider?
Yes. We can send your records once you complete and sign a Release of Information (ROI) form authorizing the third party to receive them.

Q: Is there a fee for requesting my records?
A standard processing fee of $25 will apply.

SECTION B: CONTACTING OUR LICENSING BOARD

If you have questions or concerns about the licensure or professional/medical conduct of any licensed professional, you may contact their licensing board for more information or to share your concerns. Licensing boards exist to regulate professional services.

SECTION C: FILING A CONSUMER COMPLAINT

If you have concerns about our business practices or believe you have been treated unfairly, you have the right to file a consumer complaint with the Texas Office of Attorney General’s Consumer Protection Division. This office investigates complaints about unfair, deceptive, or fraudulent business practices in Texas.
